Please read and understand the following: Only the Owner, Contractor or Authorized Principle which is legally authorized to request and sign for the bond and release of information can apply for a bond quote request. If we offer you a bond quote request and you authorize us to issue the bond you must be willing to accept a second authorization of signing for the entire process to be completed. This authorization can be either by e-mail, fax or United States mail services. If you agree to such security signing you are welcome to proceed.
You may, at you option, call our office between 8:00am and 5:00pm Monday through Friday and request a phone quote instead. The options is yours. With either method, you will be required to complete the acceptance with a signature via e-mail, fax or Unites States Mail. Not all of our Indemnity Surety Carriers offer E-signature. In such cases a fax or United States Mail would be the only remaining methods.
Be aware that by completing this form you are authorizing "us" to run a Credit check for insurance / indemnity purposes and that the person completing this form has the legal authority to do so.
